Please only complete this form if you are requesting a new MY CARE account for yourself and you hold a medical record with Royal Devon University Healthcare NHS Foundation Trust or Torbay and South Devon NHS Foundation Trust. Accounts cannot be created on behalf of another person - any account created in this way will be deactivated.
You must be aged 16 and over to sign up via this form.
If you are a parent or guardian requesting proxy access to MY CARE for a child under 16, this can be discussed at their next appointment with a clinician. You can find more information about proxy access on your Trust’s MY CARE page:
You will need your NHS number to complete the sign-up process. If you are unsure of your NHS number this can be found on a recent hospital letter or by using the NHS online service: Find your NHS number - NHS (www.nhs.uk)
Once submitted we will match the details with those in your medical record. If everything matches we will immediately create your account.
If you need to verify any details, a member of the MY CARE Helpdesk team will contact you. This process may take up to 5 working days.
